Not acute in routine care — the exceptions are myxedema coma (hypothermic, obtunded hypothyroid) and thyroid storm (fever, tachycardia, agitation): both are emergencies.

What you do
  • Backwards from intuition: HIGH TSH = LOW thyroid function
  • Levothyroxine: give on an empty stomach, same time daily, apart from calcium/iron
