How it works

Blocks vitamin K recycling → the liver can't finish clotting factors II, VII, IX, X. Takes 3–5 days to work fully.

Monitor
  • INR before today's dose, every time — therapeutic usually 2–3
  • Every new med (especially antibiotics) moves the INR — anticipate a recheck
  • Antidote: vitamin K; 4-factor PCC/FFP for serious bleeding
🎓 Patient teaching

CONSISTENT vitamin K intake — don't binge or quit greens, keep it steady. Same time daily. Medical alert bracelet. Report dark stools, unusual bruising, any head injury.
